Uraian Tugas Pokok dan Fungsi Struktur Organisasi

Fungsional LPMI

  1. Kepala LPMI

Kepala LPMI membantu Rektor UNIQHBA Bagu dalam menjalankan tugas pokok dan fungsi berikut:

  1. Menyusun program kerja dan anggaran Lembaga Penjamin Mutu Uniqhba
    1. Menyiapkan bahan dalam menetapkan Standar Dikti untuk menyusun dokumen kebijakan, peraturan, standar, manual, dan sasaran mutu bidang akademik;
  • Merumuskan semua Standar Dikti yang akan menjadi tolok ukur dalam penyelenggaraan pendidikan tinggi, baik pada aras perguruan tinggi, Fakultas maupun aras unit pengelola program studi mengacu pada SNPT dan Kerangka

Kualifikasi Nasioanal Indonesia (KKNI) Bidang Pendidikan Tinggi;

  • Penyiapan proses penetapan dokumen mutu SN Dikti dan Standar Dikti UNIQHBA Bagu dalam bentuk Keputusan Rektor;
  • Menetapkan pemberlakuan semua Standar Dikti tersebut dengan peraturan pemimpin perguruan tinggi berdasarkan mekanisme yang ditetapkan dalam Statuta Perguruan Tinggi;
  • Memantau dan mengevaluasi proses pelaksanaan standar Dikti dan mengkomunikasikan ke pimpinan unit yang dimonev dan pimpinan perguruan tinggi apabila ditemukan kesalahan atau penyimpangan yang dapat berakibat isi standar tidak terpenuhi, atau memperkuat pencapaian pelaksanaan standar untuk ditindaklanjuti berupa tindakan korektif atau perbaikan untuk memastikan pemenuhan perintah/kriteria/sasaran di dalam standar;
  • Mengidentifikasi kelemahan atau kendala yang dapat menghalangi pelaksanaan isi standar dan mengambil langkah yang diperlukan untuk mengatasi kelemahan atau kendala tersebut;
  • Mengkoordinasikan pelaksanaan Audit Mutu Internal (AMI)/Akreditasi Internal
  • Menganalisis hasil akhir pelaksanaan standar berdasarkan hasil audit, sehingga dapat disimpulkan, antara lain tentang efektivitas, keberhasilan, dan dampak atau outcomes dari pelaksanaan standar;
  • Memberikan saran dan rekomendasi untuk peningkatan mutu akademik dan non akademik kepada manajemen UNIQHBA Bagu.
  • Sekretaris LPMI
  1. Sekretaris LPMI bertugas membantu Ketua LPMI dalam mengkoordinasikan, merencanakan, dan mengembangkan sistem penjaminan mutu internal secara konsisten dan berkelanjutan dan memfasilitasi proses akreditasi program studi dan institusi.
  • Menyusun dokumen PPEPP SPMI UNIQHBA, yang terdiri dari:
  1. Dokumen kebijakan SPMI;
  • Dokumen manual SPMI;
  • Dokumen standar dalam SPMI;
  • Dokumen formulir yang digunakan dalam SPMI;
  • Serta semua dokumen SOP yang diperlukan untuk melaksanakan standar
  • Mengupdate  website LPMI
  • Staf Administrasi LPMI

Staf administrasi memiliki tupoksi membantu sekretaris dalam menjalankan tugas administrasi kantor sebagai berikut:

  1. Melakukan persiapan-persiapan teknis kesekretariatan untuk keperluan LPMI
  • Bertanggungjawab atas semua urusan surat menyurat LPMI
  • Mengarsipkan semua dokumen-dokumen LPMI (hard copy & soft copy)
  • Bertanggungjawab atas undangan rapat LPMI
  • Membuat notulensi dan menyediakan daftar hadir dalam setiap rapat LPMI
  • Membantu sekretaris LPMI untuk menupdate  website LPMI
  • Koordinator AMI

Koordinator AMI memiliki tupoksi

  1. Meningkatkan kapasitas auditor mutu internal (AMI)
    1. Mengkoordinir persiapan AMI,
    1. Melaksanakan AMI bersama dengan tim audit Internal UNIQHBA,
    1. Melaporan hasil audit mutu internal kepada rektor melalui kepala Lembaga untuk melakukan pengendalian pelaksanaan standar Dikti dan Standar UNIQHBA.
    1. Khusus hasil AMI institusi disampaikan kepada Ketua Yayasan dan atau Senat Universitas
  • Koordinator PMI

Koordinator PMI memiliki tupoksi untuk

  1. Mengkoordinir revisi standar Dikti dan Penyusunan Standar UNIQHBA
    1. Mengkoordinir persiapan penetapan standar,
    1. Mengkoordinir pelaksanaan standar,
    1. Melaksanakan monitoring dan evaluasi standar akademik dan Non akademik.
    1. Melaporkan  hasil monitoring dan evaluasi kepada rektor melalui kepala Lembaga untuk ditindaklanjuti jika menunjukkan perlu tindakan koreksi.
  • Koordinator PME

Koordinator PME memiliki tupoksi

  1. Mengkoordinasikan persiapan akreditasi meliputi persiapan data, bukti fisik/dokumen, pengisian instrumen LKPS/LKPT
    1. Mengkoordinir pelaksanaan proses akreditasi
    1. Mengkoordinir tindak lanjut rekomendasi asesor  
    1. Pelaporan proses akreditasi eksternal institusi dan program studi oleh BAN PT atau LAM maupun lembaga akreditasi internal lain  di lingkup UNIQHBA Bagu.
  • Unit Pengendali Mutu Fakultas

Unit pengendali mutu fakultas adalah komponen pelaksana implementasi SPMI pada tingkat fakultas yang dalam pelaksanaannya melekat pada pengelola fakultas dibawah komando Wakil Dekan dan dibawah koordinasi Koordinator PMI LPMI UNIQHBA. Sedangkan pelaksanaan AMI dan AME dilakukan secara terintegrasi dibawah koordinasi Koordinator AMI dan AME LPMI UNIQHBA.

  1. Melaksanakan penjaminan mutu bidang akademik dan non akademik di Fakultas dengan berkoordinasi kepada LPMI UNIQHBA Bagu.
  • Mengevaluasi hasil laporan mutu program studi bersama LPMI UNIQHBA.
  • Memberikan saran dan rekomendasi untuk peningkatan mutu akademik dan non akademik kepada manajemen fakultas melalui

LPMI UNIQHBA.

  • Gugus Pengendali Mutu Prodi

Unit pengendali mutu program studi adalah komponen pelaksana implementasi SPMI pada tingkat program studi yang dalam pelaksanaannya melekat pada pengelola program studi dibawah komando koordinator program studi dan dibawah koordinasi Koordinator PMI LPMI UNIQHBA. Sedangkan pelaksanaan AMI dan AME dilakukan secara terintegrasi dibawah koordinasi Koordinator AMI dan AME LPMI

Sedangkan pelaksanaan AMI dan AME dilakukan secara terintegrasi dibawah koordinasi Koordinator AMI dan AME LPMI UNIQHBA.
